Community Health Centers

Not every patient should travel hundreds of kilometres for care. Community clinics keep care close, integrated, and continuous.

The role of community centres

Community centres deliver primary care, antenatal and child health, vaccination, chronic-disease management, and first-line emergency stabilisation. They refer up the network when more advanced care is needed.

Integration matters

Stand-alone clinics can underperform when disconnected from referral pathways and supply chains. Each NFAHS centre is integrated with the regional teaching hospital — sharing electronic records, supply, supervision, and continuing education.

Telehealth-enabled

Telemedicine extends the reach of specialists at the hospital level into every community centre, letting frontline staff consult on complex cases without patients travelling.
